1. Acceptance of Terms
By accessing or using this website (selfdeterminedrelationships.com), you agree to be bound by these Terms & Conditions ("Terms"), our Website Privacy Policy, and all applicable laws and regulations. If you do not agree to these Terms, please do not use this website.
This website is intended for individuals 18 years of age or older. Minors may access this website only with the active involvement and consent of a parent or legal guardian, who by permitting such access agrees to these Terms on the minor's behalf.
These Terms apply to your use of this website only. The clinical relationship between you and your therapist, once established, is governed by a separate informed consent and services agreement.
2. Definitions
- "ISDR," "we," "us," or "our" refers to the Institute for Self-Determined Relationships, PLLC, its founders, clinicians, employees, and agents.
- "Site" refers to the website located at selfdeterminedrelationships.com and all associated pages.
- "Services" refers to clinical therapy, counseling, and related services offered by ISDR and its clinicians.
- "Visitor" refers to any person who accesses the Site without an established clinical relationship.
- "Client" refers to any person who has executed a formal services agreement with ISDR and is actively receiving clinical services.
3. No Therapist-Client Relationship
Using this website, reading its content, sending an email, filling out a contact form, or scheduling a consultation does not create a therapist-client, counselor-client, or any other professional relationship with ISDR or any of its clinicians.
A clinical relationship is established only when both parties have executed a formal informed consent and services agreement and clinical services have formally commenced. Until that point, no professional duty of care exists and no confidentiality obligation attaches under HIPAA or state law.

6. Clinical Services — Separate Agreement
ISDR offers individual therapy, relational or couples therapy, family therapy, and related services such as workshops, trainings, supervision, and consultations. Descriptions of these services on this website are provided for informational purposes only and do not constitute a guarantee of availability, outcome, or suitability for any particular individual.
All clinical services are governed by a separate informed consent and services agreement, which you will receive, review, and sign before your first appointment. That agreement — not these Terms — governs the clinical relationship, confidentiality, cancellation policies, fees, and your rights as a client.
ISDR clinicians are licensed in specific states. Services may only be provided to clients located in a state where the treating clinician holds an active license. You are responsible for confirming your clinician's licensure in your state of residence prior to beginning services.
7. No Guarantee of Outcomes
We do not guarantee specific therapeutic outcomes. Results vary based on individual circumstances, personal effort, motivation, the nature and severity of presenting concerns, and many other factors outside our control. Research supports therapy as an effective approach for many people. Therapy is a collaborative process that requires your active participation and commitment. Any statements or examples on this website represent individual experiences and are not intended to imply that you will achieve the same or similar results. We will exercise our best professional efforts, skills, and care — however, we cannot guarantee the outcome of those efforts, and any comments regarding expected results are expressions of professional opinion only.
8. Fees, Payment & Good Faith Estimates
Session fees vary by clinician, service type, and session length. Current fee schedules are provided during the intake process and may be updated from time to time. ISDR does not guarantee that any particular insurance plan will cover Services.
No Surprises Act — Your Right to a Good Faith Estimate: Under federal law (the No Surprises Act, effective January 1, 2022), you have the right to receive a Good Faith Estimate of expected costs for healthcare services, including mental health services, before your first appointment. You are entitled to this estimate if you are uninsured or if you choose to pay out-of-pocket and decline to use insurance.
- Your Good Faith Estimate will include expected charges for services, including session fees and any anticipated additional costs.
- If you receive a bill that exceeds your Good Faith Estimate by $400 or more, you may dispute the bill.

Insurance billing is handled according to your individual benefits plan. ISDR is not responsible for insurance denials, benefit determinations, or any portion of fees not covered by your plan. You are ultimately responsible for all fees not covered by insurance.
9. Appointment Scheduling & Cancellation
Appointments may be scheduled through this website, our client portal, or by contacting us directly. Appointment availability is subject to clinician scheduling and may change without notice.
Cancellation policy: Once a clinical relationship is established, specific cancellation and no-show policies will be outlined in your informed consent agreement. ISDR requires a minimum of 24 hours' notice to cancel or reschedule an appointment without incurring a late cancellation fee. Late cancellations and no-shows are charged the full session fee; for clients using insurance, the therapist's out-of-network rate applies, as late cancellations are not covered by insurance.
Scheduling a consultation through this website does not guarantee the commencement of a clinical relationship. ISDR reserves the right to determine whether to accept any client into services based on clinical appropriateness, scope of practice, and availability.
10. Telehealth Services
ISDR offers telehealth therapy sessions in addition to in-person services, where available. Telehealth is subject to the following conditions:
- Telehealth sessions require a stable internet connection and a private, secure location on your end. ISDR is not responsible for technical failures on the client's side.
- Telehealth may not be appropriate for all clients or all clinical situations. Your clinician will advise you if telehealth is not clinically appropriate for your needs.
- State licensure requirements apply: your clinician may only provide telehealth services to you if you are physically located in a state where they hold an active license at the time of the session. You are responsible for notifying your clinician if you will be in a different state during a scheduled session.
- Telehealth platforms used by ISDR are HIPAA-compliant. Full details are provided in your Telehealth Disclosure and Consent document, which you will receive before your first telehealth session.
- In the event of a technology failure during a session, your clinician will attempt to reconnect via phone. Please ensure your clinician has your current phone number on file.

11. Digital Communication Policy
Standard email and website contact forms are not encrypted end-to-end and are not fully secure means of communication. Please do not send sensitive personal health information through this website's contact form or via standard email.
Before using email or text to communicate with the Practice, please carefully consider who else may have access to your devices or accounts — partners, family members, employers, shared devices, lock-screen previews, and so on. For any communication involving clinical content, please use the secure client portal once a clinical relationship is established.
Contact forms and general email ([email protected]) are appropriate for: scheduling inquiries, general questions, and non-sensitive administrative matters.
Secure client portal is the appropriate channel for all confidential clinical communications once a clinical relationship is established. ISDR uses a HIPAA-compliant client portal through SimplePractice.
Social media: ISDR maintains professional social media accounts for educational and outreach purposes. ISDR clinicians will not accept friend or follow requests from current or former clients on personal social media accounts. We ask that clients not contact clinicians through social media platforms, as these are not secure channels. Liking, sharing, or commenting on our public content does not create or imply a clinical relationship.
By contacting us through this website, you acknowledge and accept the inherent limitations of internet-based communications.
12. Confidentiality & Privacy
ISDR takes the privacy of all website visitors and clients seriously. Our data practices are described in detail in two documents:
- Website Privacy Policy — governs information collected through this website (contact forms, cookies, analytics).
- Notice of Privacy Practices — governs Protected Health Information collected in the course of clinical services, as required by HIPAA.
These website documents are not the exclusive source of ISDR's data and privacy practices. Clients also receive additional consent, authorization, and data practice disclosures through intake paperwork, informed consent agreements, and other clinical forms provided at the outset of services. Those documents govern the clinical relationship and are complementary to — not replaced by — the policies linked above.

20. Right to Refuse or Terminate Services
ISDR reserves the right, in its sole discretion, to refuse services, limit access to this website, or terminate any established clinical relationship in accordance with applicable law and professional ethical standards. Grounds for termination of clinical services, and the process for doing so, are detailed in the informed consent and services agreement.
ISDR also reserves the right to suspend or terminate your access to this website at any time, without notice, for conduct that violates these Terms or that ISDR believes is harmful to other users, the public, or ISDR's interests.
21. Governing Law & Dispute Resolution
These Terms are governed by and construed in accordance with the laws of the State of Utah, without regard to conflict of law provisions. ISDR also provides services to residents of Kansas and Missouri; residents of those states may have additional rights under applicable state law.
Informal resolution: Before initiating any formal legal proceeding, you agree to first contact ISDR in writing at [email protected] to describe the nature of the dispute and the relief sought. The parties will attempt in good faith to resolve any dispute informally within 30 days of notice.
If informal resolution fails, any dispute arising from these Terms or your use of this website shall be subject to the exclusive jurisdiction of the state and federal courts located in Utah, and you consent to personal jurisdiction in such courts.
22. Severability
If any provision of these Terms is found by a court of competent jurisdiction to be invalid, illegal, or unenforceable, that provision shall be modified to the minimum extent necessary to make it enforceable, or severed from these Terms if modification is not possible. The remaining provisions shall continue in full force and effect.
